Do you know a healthcare provider who would be a good fit at iCare Health Solutions? We’re always looking for talented and experienced providers to add to our network. Please complete the form below to nominate a provider not currently participating in the iCare network.

Nominating a provider does not guarantee participation in the iCare network.

Once the form is submitted, our network recruitment team will review the information and contact the provider to gauge their interest in participating with iCare. If successful, the recruiting, contracting, and credentialing process can take up to 90 days to complete.
